Pulmonary Atresia with Intact Ventricular Septum: Management Options and Decision-making

چکیده
Pulmonary atresia with intact ventricular septum (PA-IVS) is a complex congenital heart malformation with a diverse set of anatomical and clinical findings. The incidence is 4.1 per 100,000 live births and is less than 1% of all congenital heart disease. During embryogenesis, PA-IVS is postulated to occur after development of ventricular septum which is later than the development of PA with ventricular septal defect. Every case of PA-IVS poses a considerable challenge to the pediatric cardiologist and cardiovascular surgeon. Although echocardiography is often the first line tool in cardiac imaging, cardiac catheterization is the gold standard for diagnosing PA-IVS and describing the important anatomical features that determine the plan of treatment. This article will focus on the management options and decision making from the interventional cardiologists point of view. *Corresponding author: Henry Burkholder, Division of Pediatric Cardiology, University of Texas Houston Medical School, 6410 Fannin Street, UTPB–425, Houston, TX 77030, USA, Tel: 713-500 5738; Fax: 713-500 5751; E-mail: [email protected] Received February 21, 2012; Accepted December 17, 2012; Published December 19, 2012 Citation: Burkholder H, Balaguru D (2012) Pulmonary Atresia with Intact Ventricular Septum: Management Options and Decision-making. Echocardiographic assessment of neonates with pulmonary atresia and intact ventricular septum.
In this prospective study, 27 consecutive neonates suspected to be suffering from pulmonary atresia and intact ventricular septum underwent detailed two-dimensional echocardiographic examination before cardiac catheterization. Of the 27 neonates 25 had pulmonary atresia and intact ventricular septum and the remaining 2 had "functional pulmonary atresia" secondary to severe Ebstein's anomaly of ...
متن کاملPrenatal Diagnosis of Pulmonary Atresia with Intact Ventricular Septum
Hypoplasia of the right ventricle is uncommon as an isolated entity. It may result from tricuspid atresia, but is most commonly secondary to pulmonary atresia with intact ventricular septum. Pulmonary atresia with intact ventricular septum is rare, accounting for 1-3% of cases of congenital heart disease. It occurs in 0.1 to 0.4 in 10,000 live births. Pulmonary atresia with intact ventricular s...
متن کامل[Pulmonary atresia with intact ventricular septum associated with severe aortic stenosis].
Pulmonary atresia with intact ventricular septum is the complete obstruction of the right ventricular outflow tract due to pulmonary valve atresia in the absence of ventricular septal defect. Pulmonary flow is dependent on the ductus arteriosus. Other morphological anomalies are also present. Aortic stenosis in association with pulmonary atresia with intact ventricular septum is extremely infre...
متن کاملPulmonary atresia, "intact ventricular septum", and aortopulmonary collateral arteries.
In muscular pulmonary atresia, major aortopulmonary collateral arteries are characteristic of pulmonary atresia with ventricular septal defect and are rarely seen in pulmonary atresia with intact ventricular septum. Two unusual cases of muscular pulmonary atresia are reported, one with an intact septum and one with a perimembranous ventricular septal defect, closed in utero by aneurysmal tricus...
متن کاملBiventricular repair of pulmonary atresia with intact ventricular septum and severely hypoplastic right ventricle: a case report of a minimum intervention surgical approach
BACKGROUND In patients who have pulmonary atresia with an intact ventricular septum and severe right ventricular hypoplasia, biventricular repair is considered to be impossible and multiple interventions are generally required for definitive repair. CASE PRESENTATION An initial palliative procedure was performed in a 1-month-old boy to promote right ventricular development by pulmonary valvec...
